Versed
ADULT:
Seizures / Cardioversion / Psych / Behavioral Emergencies / Excited Delirium / Pacing / Sedation for Intubation
10 mg IM - Seizures. (IM preferable for initial dose)
5 mg IV - Seizers (If IV is present already)
5 mg slow IV BHO - Cardioversion.
5 mg IM / IN, (IN: 1/2 dose each nostril, Max 1cc per nostril per dose) BHO – Cardioversion.
PEDS:
Seizures
0.2 mg/kg IM, titrated to effect (10 mg maximum). BHO for additional doses.
or
0.1 mg/kg slow IN / IV / IO (if unable to deliver IM or IV/IO already present), titrated to effect (5 mg maximum). May repeat once.
Cardioversion / Psych
Over 12 months:
0.1 mg/kg slow IM / IN (1 mg/min), titrated to effect (5 mg maximum). May repeat once.
Under 12 Months: BASE HOSPITAL ORDER
0.1 mg/kg slow IM / IN (1 mg/min), titrated to effect (5 mg maximum). BHO
BHO - Cardioversion / Psych / Under 1 y/o age
***DO NOT ADMINISTER IV FOR PEDS***
INDICATIONS:
- On-going and/or recurrent seizure activity.
- Psychiatric/behavioral emergencies (severe agitation).
- Suspected Stimulant Intoxication / Excited Delirium.
- Intolerance/Anxiety with transcutaneous pacing.
- BP > 90 systolic.
- Sedation for cardioversion of SVT / VT (BHO).
- Sedation for Intubation (clenched jaw, coughing or choking), BP > 90.
COMMENTS:
- Carefully monitor respiratory status during administration, support ventilation as necessary.
- May be potentiated when combined with ETOH or other CNS depressants.
- Provides an amnesiac effect, NOT a sedative effect, therefore, will not necessarily produce lethargy.
- Consider reducing IM or IV dose of patients > 60 years.
- In Status Epilepticus: Duration of action after IV dose is up to 2 hour.
